Only when this form has been completed and returned, will confirmation be made to the above named artist(s) of their inclusion in the event. Any information contained here will be the full and final authority of agreement.

Any conversations with any party pertaining to the event, not reflected here, will not be considered binding.

Anyone not arriving for set up and/or missing their performance slot may removed from the programme at the stage managers discretion.
